Mission Statement and Purpose

The purpose of the Green Brook Township Public Schools Athletics Program (grades 6–8, with the exception of cross country which is also available for grade 5) is to initiate, stimulate, and improve opportunities for students to expand their learning through competition; and to promote and develop leadership, sportsmanship, fitness, school spirit, and athletic excellence through mass participation in various athletic disciplines.

NJ UPDATED MEDICAL SPORTS FORMS

The following medical forms need to be completed by clicking the links below:

1. Please submit the New Jersey Department of Education Medical Eligibility Form, completed and signed by your physician, to the school nurse to verify that your child passed a sports physical within the last 365 days and is medically cleared to participate. Physical Examination & Medical History Forms should be kept by the healthcare provider who completes the PPE.  NOTE: A new Medical Eligibility Form must be submitted to the health office within two weeks of the expiration date should your child’s physical expire before the end of the season. 

2. The Health History Form is only required if your child's sports physical date is over 90 days old. It indicates to us that nothing has changed medically with your child since you last submitted the sports physical. The health history form must be submitted to the school nurse for every sports season if your physical is over 90 days in order to participate.  

3.  The Mandated Athlete Information Forms need to be completed once per year & submitted electronically!
