Mission Statement and Purpose
The purpose of the Green Brook Township Public Schools Athletics Program (grades 6–8, with the exception of cross country which is also available for grade 5) is to initiate, stimulate, and improve opportunities for students to expand their learning through competition; and to promote and develop leadership, sportsmanship, fitness, school spirit, and athletic excellence through mass participation in various athletic disciplines.

NJ UPDATED MEDICAL SPORTS FORMS
The following medical forms need to be completed by clicking the links below:
Physical Examination & Medical History Forms should be kept by the healthcare provider who completes the PPE and the Medical Eligibility Form will need to be submitted to the GBMS school nurse. Click the following links to obtain these forms:
1. Only the completed Medical Eligibility Form should be completed & signed by a physician. This form needs to be submitted online/hard copy to the school nurse which states that your child has had a sports physical that was completed within the last 365 days and is medically cleared by your child's home physician to participate in sports. NOTE: A new Medical Eligibility Form must be submitted to the health office within two weeks of the expiration date should your child’s physical expire before the end of the season.
2. The Health History Form is only required if your child's sports physical date is over 90 days old. It indicates to us that nothing has changed medically with your child since you last submitted the sports physical. The health history form must be submitted for every sports season if your physical is over 90 days in order to participate.

HEALTH HISTORY FORM
If your child already has a sports physical on file approved by the school physician in the last year, please make sure to fill out the Health History Update Form that can be submitted online/hard copy to the school nurse. This form indicates to us that nothing has changed medically with your child since you last submitted the sports physical. The health history form must be submitted for every sports season in order to participate.
